AMENDING A VITAL RECORD

Sometimes a person may find that the information on the official birth record is not correct or information is missing. If this is the case, it is possible to have the official record corrected or information added. There is a fee of $10.00 for any correction to a birth record if the correction is not done within 14 days of the file date.

Corrections are done at the Town Clerks Office in the town which the birth occurred during regular business hours.

Within 6 months of the birth, please bring the following:

• a photo ID

After 6 months of the birth, please bring the following:

• A photo ID
• Two (2) proofs of the correction
